All documents must be original or certified — photocopies and notarized copies are not accepted.

Primary identification — US citizens: certified US birth certificate, valid US passport/passport card, Consular Report of Birth Abroad, Certificate of Naturalization or Certificate of Citizenship. Immigrants and non-immigrants: valid USCIS documents (Green Card, I-551, I-94, I-766, etc.)
Proof of Social Security number — or, if you've never been issued an SSN, an approved secondary ID
Two documents showing your Florida residential address (utility bill, bank statement, lease, mail from government agency, etc.)
Name-change document if your name changed since your primary ID was issued (certified marriage certificate, divorce decree, or court order)
Parental consent form signed by a parent or legal guardian (applicants under 18)
TLSAE certificate — required for first-time Florida applicants who have never held a driver license anywhere
Proof of school enrollment (under 18 — compliance with school-attendance requirements)
Payment for the applicable fee
Glasses or contacts if you need them for the vision exam (20/40 in each eye required)
