Primary ID: certified birth certificate (with raised seal) OR valid U.S. passport / passport card
Social Security number — card, W-2, or pay stub with full SSN
Two proofs of Georgia residency — utility bill, bank statement, signed lease, or tax return (addressed to you at a Georgia address)
Proof of U.S. citizenship OR lawful presence (naturalization certificate, I-551 permanent resident card, or valid I-766 EAD)
Under 18: parent, legal guardian, or responsible adult present to sign the application + Responsible Adult Affidavit + school-enrollment proof (DS-1, diploma, or GED) + ADAP completion certificate
For Class D (age 16–17): proof of Joshua's Law completion — driver education course + supervised driving affidavit (40 hours, 6 at night)
Payment — DDS accepts cash, Visa, Mastercard, Discover, American Express, and mobile pay (no checks or money orders)
